I was driving slowly and braked at a safe distance, but it was so icy that I collided into the car ahead of me. Was the accident my fault?

The reality is, if you ran into the car you were following, then you were not traveling behind it at safe distance. It is indeed your fault.

The exception to this is known as the black ice defense, meaning that a reasonable and prudent driver could not have been expected to anticipate the presence of ice, and it occurred unexpectedly. This type of situation is rare, however you can talk to your own insurance adjuster about it.
